How losartan brings blood pressure down
Losartan is an angiotensin II receptor blocker (ARB). Angiotensin II is the body's own vessel-tightening hormone, and losartan blocks the receptor it docks with, so the vessels stay relaxed and the pressure inside them drops. Blood pressure that runs high for years damages the heart, brain, kidneys and eyes; holding it down cuts that risk. Since high blood pressure seldom gives warning signs, how you feel says little about whether it is controlled.
It is used for two things: high blood pressure in adults, and lowering the chance of a stroke in people whose high blood pressure has thickened the left side of the heart (left ventricular hypertrophy). It is not used in pregnancy.
Taking losartan each day
One tablet, once a day, at a time you can keep to. Food does not change how much you absorb. Most people begin at 50mg; a 25mg start suits those already on a water tablet (diuretic) or with liver problems, and 100mg is the top strength if pressure is still high. The effect grows over the first few weeks, so a single reading early on does not show where you will settle.
Side effects, common and rare
Light-headedness and tiredness turn up most often at the start, especially on standing up, and usually ease as the body adjusts. Losartan can raise the potassium level in your blood and shift kidney readings; both show up on a blood test. A dry cough, the reason some people move off ACE inhibitors, is uncommon here. Swelling of the lips, tongue or throat is rare and needs urgent treatment.
Other medicines that matter
Potassium supplements, potassium-based salt substitutes and potassium-sparing water tablets (spironolactone, amiloride, triamterene) can push blood potassium too high. Taken regularly with losartan, ibuprofen or naproxen can undo some of its pressure-lowering work and put the kidneys under strain. Lithium levels can rise.
