Albendazole 400mg tablets are an anthelmintic (a medicine that kills parasitic worms) used for tapeworm, pinworm and hookworm infections, sold in packs of 30 to 360 tablets.
Albendazole clears a group of worm infections that live in the gut and, with longer courses, worms that have settled elsewhere in the body. Threadworm (pinworm), roundworm, whipworm and hookworm are the everyday ones: the worms behind itching around the back passage, tummy pain, and the anaemia that heavy hookworm brings. Tapeworm in the gut is treated with it as well.
It also has a place in infections where larvae travel beyond the bowel: hydatid disease (cysts in the liver or lungs from dog tapeworm) and neurocysticercosis (tapeworm cysts in the brain). Children with giardiasis are sometimes given it, and control programmes for lymphatic filariasis use it together with other deworming medicines.
The tablet is a benzimidazole, the same family as mebendazole. Once inside a worm's cells, albendazole binds to tubulin, a protein the worm uses to build its internal scaffolding and to divide. Block that and the worm can no longer take in glucose; its energy reserves drain, it loses its hold on the gut wall and dies. Human cells are much less affected at these doses, so the work falls on the worm.
Both products hold 400mg albendazole tablets. A gut worm such as threadworm or hookworm usually needs one tablet and no more, and that tablet absorbs best alongside food with some fat in it. Worms that have spread to the liver, lungs or brain mean a much longer treatment, often a tablet morning and evening for about four weeks and then a repeat course after a pause if the infection needs it.
Headache, nausea, stomach pain and dizziness are the usual ones, and they ease off. Courses that run for weeks can push liver enzyme levels up on a blood test and thin the hair, which recovers after treatment stops. A rash, or swelling of the face and lips, is rare and needs urgent treatment. Among medicines, cimetidine (for stomach acid) and dexamethasone (a steroid) can raise albendazole levels in the body.
