Atorvastatin is the active ingredient in Lipitor, for years one of the best-selling medicines in the world, and it is now sold as a generic under its own name as well. The mechanism is simple enough to picture. The liver builds cholesterol with an enzyme called HMG-CoA reductase. Atorvastatin holds that enzyme back, and as the liver's own output drops, it takes more LDL out of the blood.
Both are tablets. Atorvastatin comes in 10mg, 20mg, 40mg and 80mg, the whole range under the ingredient's name. Lipitor is generic atorvastatin again, with tablets stopping at 40mg. The ingredient is the same; the difference is the name on the pack and the 80mg tablet, which only the plain listing has.
Atorvastatin is a once-a-day tablet, any time of day, with or without food. It is a long-term medicine, because cholesterol rises again when it is stopped, and high cholesterol itself causes no symptoms to tell you it is working.
Common effects are diarrhoea, heartburn, gas, joint pain and, for some people, forgetfulness or confusion. The serious ones are muscle pain, tenderness or weakness that cannot be explained, particularly with a fever or dark urine, and yellowing of the skin or eyes. These need urgent treatment. It is not used in pregnancy or breastfeeding, or in active liver disease.
Interactions are worth knowing before ordering. Large amounts of grapefruit juice, certain antibiotics such as clarithromycin, antifungals such as itraconazole, ciclosporin and some HIV medicines raise atorvastatin levels. Niacin and fibrates raise the chance of muscle problems. Drinking more than two alcoholic drinks a day adds to the risk.
Simvastatin is listed up to 40mg, rosuvastatin stops at 20mg, and atorvastatin reaches furthest, to 80mg. Ezetimibe, bempedoic acid, fenofibrate and Questran sit alongside them in the cholesterol category.
