Azithromycin sits in the azalide branch of the macrolide family. It reaches high concentrations inside tissues and cells, which suits infections that hide there. That is how it earned its place for community-acquired pneumonia, bronchitis flare-ups, sinusitis, tonsillitis and throat infections, middle-ear infections in children, skin infections and chlamydia. Less often it is used for Lyme disease, whooping cough and travellers' diarrhoea.
It binds to the 50S part of the bacterial ribosome and blocks protein manufacture. Growth stops, and the immune system finishes the infection. Because the drug stays in infected tissue long after the last tablet, a course can be as short as three days, or even one dose.

Zithromax is the brand name version, offered in 100mg, 250mg, 500mg and 1000mg, so it also gives a lower-strength option. Zithromax Dispersible is a 100mg tablet that is stirred into water instead of swallowed whole, which suits anyone who finds tablets hard to swallow.
One dose a day is the usual rhythm, and food makes no difference to whether it works. Antacids containing aluminium or magnesium blunt absorption, so space them from the tablet. If a day slips by, the tablet goes down on remembering, unless the next is nearly due.
Gut effects are most common: loose stools, nausea, stomach cramps and vomiting. Dizziness and headache come next. Swelling of the face, yellowing of the skin or eyes, or a pounding or irregular heartbeat are signs of a serious reaction and need urgent treatment. People who already have a heart rhythm problem known as long QT, or who take other medicines with the same effect, have more reason to be careful, and so do those with liver disease. Warfarin is another medicine whose effect can shift. Anyone allergic to a macrolide antibiotic should not take it. Because the three listings share one ingredient, they should not be taken together; choose one and stay with it for the whole course.
