Candesartan cilexetil 4mg, 8mg and 16mg tablets for high blood pressure and heart failure — an angiotensin II receptor blocker, in packs from 10 to 360 tablets.
Candesartan cilexetil is the ingredient in the tablet; the body changes it into candesartan, which is the form that works. It belongs to the angiotensin II receptor blockers (ARBs), a group of blood-pressure medicines. The body's own angiotensin II narrows arteries and raises pressure; candesartan occupies the receptor that hormone would use, so arteries stay more open and the heart meets less resistance when it pumps. Pressure eases over days and weeks, not within minutes of a tablet.
Adults take it for two main reasons: high blood pressure, including the everyday kind with no obvious single cause, and heart failure in which the left pumping chamber is weak. In heart failure it is commonly given alongside an ACE inhibitor, or by itself when an ACE inhibitor is unsuitable. It is not used from the second trimester of pregnancy onwards, or in severe liver disease with cholestasis (blocked bile flow).
Candesartan is sold as ordinary tablets in 4mg, 8mg and 16mg. The figure on the box is just the amount of candesartan cilexetil in that tablet; all three strengths are the same medicine. Doses tend to start low and rise in steps when pressure stays high, so 4mg or 8mg is often the first tablet and a larger one comes later. It is part of the heart and blood pressure range.
Most people keep the candesartan tablet beside whatever they already do each morning, though evening suits equally well and meals do not get in the way. The tablet is designed to last about 24 hours. Leave it off for several days running and pressure is simply back where you began.
Dizziness, headache, tiredness and cold-like symptoms are the ones people report most, and light-headedness on standing is more likely early on or after a dose rise. Blood potassium can drift upward, so tests are often arranged. Rarely, the face or throat swells and breathing becomes difficult, or the skin and eyes turn yellow; both need urgent treatment. Ibuprofen and similar anti-inflammatory painkillers can dull the pressure-lowering effect and put extra strain on the kidneys. Lithium can accumulate, and potassium supplements, potassium-sparing water tablets or potassium-rich salt replacements can raise potassium further.

The fall in pressure is gradual. A single tablet does not change a reading much, and the full benefit usually shows after a few weeks of daily doses. Checks are commonly done a few weeks in, once the dose has had time to settle.
A dry cough is uncommon with candesartan. It blocks angiotensin II at the receptor instead of through the enzyme that ACE inhibitors act on, so a cough that appeared on an ACE inhibitor is unlikely to return with this medicine.
The tablets differ only in how much candesartan cilexetil they hold: 4mg, 8mg or 16mg of one and the same medicine. Treatment commonly begins with a smaller tablet, and a larger one follows only if readings stay high after a spell on the lower dose.
A single ibuprofen is unlikely to matter, but a run of daily doses can weaken candesartan and tax the kidneys, and naproxen or diclofenac behave the same way. Paracetamol leaves both blood pressure and kidneys alone, which makes it the usual choice for everyday aches.
Not to the minute. Pick a point in the day you can repeat, such as with breakfast or last thing at night, and tie the tablet to something you already do so it sticks. A day missed matters more than a tablet taken two hours late.
