NeoMercazole is a carbimazole tablet for an overactive thyroid (hyperthyroidism). It comes as generic 5mg and 10mg tablets in packs of 130 or 260.
An overactive thyroid makes the gland in the neck release far more hormone than the body needs. That shows up as a fast or pounding heartbeat, sweating, shaky hands, weight loss despite a good appetite, irritability and broken sleep. Carbimazole brings that output down. It is used for Graves' disease, where the immune system drives the gland too hard, and for other causes of an overactive thyroid. It also settles hormone levels before thyroid surgery or radioiodine treatment, and is sometimes given alongside radioiodine.
Swallow the tablets with a glass of water, either with a meal or between meals, and keep to roughly the same time each day. Treatment usually opens with a larger daily dose and is trimmed back to a smaller maintenance amount once blood levels settle. Some people take the whole dose in the morning, others split it into two.
Nothing dramatic happens in the first days. The gland holds a reserve of ready-made hormone, so levels drift down over a few weeks and symptoms fade gradually. If a dose is missed, take it later the same day; when the next dose is nearly due, leave the missed one and carry on as normal.
Feeling sick, stomach upset, headache, aching joints, itching and a mild rash are the effects people report most often, and they usually appear early. A metallic taste in the mouth sometimes turns up too. A spreading rash, or swelling of the lips and face, is not something to sit on.
The reaction that matters most is a fall in white blood cells, the cells that fight infection. A sore throat with a fever, mouth ulcers or yellowing of the skin can be the first sign of a blood or liver problem and needs urgent attention.
Thyroid hormone levels are checked every few weeks at first, then less often once they are steady. A full blood count is usually checked as well, because of the small risk to white cells. Dose changes are made against those results rather than against how you feel on a given day.

Carbimazole lowers thyroid hormone production over a few weeks rather than days. The thyroid holds a reserve of finished hormone, so the first sign of progress is usually a blood test rather than a sudden change in how you feel; the racing heart and sweating ease as that reserve runs down.
Carbimazole does not add weight by itself. Weight often drops while the thyroid is running fast, and it tends to come back as levels return to normal, which is expected rather than a side effect. Weight that climbs quickly, or swollen ankles, is worth checking.
They are two forms of the same treatment. Carbimazole is inactive as swallowed — the body turns it into methimazole, and methimazole is what blocks the thyroid from building new hormone. Tablets sold as methimazole are used in countries where carbimazole is not the standard form.
For Graves' disease a course commonly runs for about 12 to 18 months, with the dose lowered as thyroid levels settle. Some people need a small maintenance dose for longer if the thyroid stays overactive.
