Clarithromycin 250mg and 500mg tablets are a macrolide antibiotic used for bacterial chest, throat, sinus and skin infections, stocked here in packs of 28 or more.
Clarithromycin belongs to the macrolide family of antibiotics. It interferes with the way bacteria assemble the proteins they need, so they cannot multiply, and the infection is then left for the body's own defences to clear. It is used for chest infections including bronchitis and pneumonia, for sinus and throat infections, and for infections of the skin. Clarithromycin also has a place in combination therapy that clears Helicobacter pylori, the bacterium behind many stomach ulcers, and in people with a weakened immune system it treats and prevents a lung infection called Mycobacterium avium complex (MAC).
Take a tablet with a drink of water. Food makes no difference to how it works, so a dose can go with a meal or in the gap between meals. Adults usually take 250mg twice a day, or 500mg twice a day when the infection is more stubborn, with the doses about 12 hours apart. Some infections are treated with three doses a day, 8 hours apart. A course generally runs from 7 to 14 days, and certain infections need several weeks. Keeping to the same times each day helps you remember. It is worth finishing the pack: bacteria that survive a shortened course are the ones that come back and resist treatment later.
The complaints people mention most are digestive — queasiness, loose stools, stomach cramps — along with a bitter or metallic taste in the mouth that often fades after the first few days. Headache is common too.
Some effects are rarer and more serious. Hearing can become muffled, or ringing in the ears can start. Liver inflammation shows as yellowed skin or eyes. Heart rhythm can be disturbed. Facial or throat swelling, a spreading rash or difficulty breathing point to a severe allergic reaction, which needs emergency care, as does diarrhoea that becomes severe, watery or bloody during or after treatment.
Clarithromycin slows down one of the liver enzymes that clears other medicines from the body, so the levels of those medicines can rise while you are taking it. Statins, warfarin, some drugs used for heart rhythm problems, certain anticonvulsants such as carbamazepine, and omeprazole are the ones that matter most. St John's wort is affected too. For that reason, another antibiotic may be chosen instead.
