Clobetasol 0.05% is a very potent topical corticosteroid cream in 15g and 30g tubes, used short-term for stubborn psoriasis, eczema and dermatitis patches that have not settled with milder creams.
Clobetasol propionate is a topical corticosteroid — a steroid rubbed into the skin rather than swallowed — and it sits at the top of the potency scale. That makes it the pick for patches of psoriasis, eczema and lichen planus (an itchy, flat-topped rash) that have not cleared with a milder steroid. Its licensed uses also cover discoid lupus erythematosus, a rash of round, scaly discs, and other stubborn steroid-responsive rashes that have resisted weaker creams.
On the skin, clobetasol quietens the immune reaction that keeps a patch red and itchy and narrows the small blood vessels beneath it, so swelling and colour ease; nothing beyond the skin it touches is treated.
Picture a patch of psoriasis on the elbow that a milder steroid has not shifted: this is the tube for it, once or twice a day, as a film thin enough to leave the skin slightly shiny rather than coated. Wash your hands afterwards unless they are the area being treated. Keep it away from the eyes and eyelids, and off broken or infected skin. A plaster, dressing or plastic wrap over the patch pushes more of the drug through the skin and adds to the risk of side effects, so leave the area open to the air. A course normally runs no longer than four weeks; if the flare is still active after that, a milder steroid takes over.
The first applications may sting, burn or itch, and the skin can feel dry; this usually settles as the flare calms. Keep going for longer and the steroid effects show up: thinning skin, stretch marks, fine red veins, and patches of changed colour. Use it over a wide area or under a dressing and enough is absorbed to cause steroid effects elsewhere in the body. It also quietens local immunity, so a skin infection — impetigo, athlete's foot, a cold sore — can take hold or spread, which is why infected skin is not treated with it.
Clobetasol 0.05% comes in 15g and 30g tubes at the 0.05% strength, and it sits with the other skin care products. A 15g tube covers a single patch through a short course; 30g is the better buy when the area is larger or the same flare tends to come back.

Twice a day is the ceiling, and some patches do well on once. A pea-sized squeeze goes a long way: the skin should end up barely shiny. Stop when the patch has cleared or the four weeks are up.
The itch tends to let up within a few days and the redness follows. Because long use thins the skin, the tube is meant for a short stretch, and it is put away once the patch has calmed.
Not usually. The face, armpits and groin have thin skin that absorbs more of the steroid and thins faster with repeated use, so these areas are treated with a milder preparation. The tube is meant for the arms, legs, trunk and scalp patches that need something stronger.
A plaster, dressing or plastic wrap over the treated skin traps the steroid and pushes more of it into the body, which raises the chance of thinning skin and other steroid effects. Leave the patch open to the air after rubbing it in.
The label covers adults, older people and children over one year. Children's skin absorbs proportionally more of a topical steroid, so treatment is limited to the affected patches and kept brief.
