What clomiphene is for
Clomiphene, also spelled clomifene, is an ovulatory stimulant: a tablet that encourages the ovaries to release an egg. It is used to treat infertility when ovulation fails — the monthly egg is not produced, or not released. Other hormone-related problems, such as irregular periods and persistent breast milk production, are sometimes treated with it as well. It is not taken during pregnancy, and it is avoided in liver disease and in unexplained vaginal bleeding.
How it works
The tablet binds to oestrogen receptors in the brain and blocks the signal that normally tells the pituitary gland to slow down. With that brake released, the pituitary puts out more follicle-stimulating hormone and luteinising hormone. Those two hormones act on the ovaries, which respond by maturing a follicle (the sac that holds an egg) and releasing the egg.
Taking a course
A first course is usually 50mg once a day for five days, starting on or around day five of the cycle; if there has been no recent period, the starting point can differ. Taking each tablet at the same time of day makes the five days easier to track, and no change to your normal diet is needed. Courses are short rather than continuous, and use beyond roughly six cycles is not advised, because longer treatment has been linked to ovarian cancer. Tablets keep well in the container they came in, closed and out of children's reach, somewhere cool and dry.
Side effects
Flushing — a sudden warm feeling — upset stomach, vomiting, breast discomfort, headache and bleeding between periods are the effects most often reported. The ovaries can enlarge, which may show up as lower abdominal pain or bloating. Less common are blurred vision, spots or flashes, double vision, stomach swelling, weight gain and shortness of breath; vision changes need prompt attention. Clomiphene also makes a multiple pregnancy more likely, since more than one egg can be released in one cycle.
