Eplerenone brings down high blood pressure, and it is also used after a heart attack to help the heart recover. Doses often start low and may be raised after about four weeks, and the full effect can take a month or more to appear. Eplerenone keeps pressure down while you take it rather than curing the underlying problem.
Potassium is the effect to take seriously. Eplerenone makes the body hold on to it, and a level that climbs too high can show up as weak or heavy muscles, tingling in the arms and legs, confusion or an irregular heartbeat. Those signs need urgent treatment.
The everyday effects are milder: headache, dizziness, diarrhoea, stomach pain, cough and unusual tiredness. Some men get breast tenderness or enlargement, and some women notice unexpected vaginal bleeding.
Potassium supplements and salt substitutes made with potassium are the first things to leave out — they add to the potassium the medicine is already holding on to. Anti-inflammatory painkillers such as ibuprofen and naproxen, aspirin, and the herbal remedy St John's wort can also affect how it works. Grapefruit juice changes the way the body clears eplerenone, so it is worth avoiding while you take the tablets.
