Finasteride is a tablet that lowers dihydrotestosterone (DHT), a male hormone made from testosterone by the enzyme 5-alpha reductase. In the scalp, DHT stops hair growth and shrinks follicles in men prone to male pattern hair loss. In the prostate, it drives the gland to enlarge, causing benign prostatic hyperplasia (BPH). Blocking the enzyme addresses both, at different strengths, and the effect lasts only for as long as the tablets continue.
The 1mg dose is the hair-loss strength. Finasteride tablets and Finpecia are both 1mg, and Propecia covers 1mg along with 5mg as generic finasteride. Packs of the 1mg tablets are the usual choice for hair loss, and each product page lists its pack sizes. The treatment is slow: at least 3 months before any change, with improvement expected within the first 12 months. It holds back loss only while you keep taking it, and hair gained is usually lost again within about a year of stopping.
The 5mg tablet. Fincar and generic Proscar come only in that strength and are listed for BPH (benign prostatic hyperplasia, an enlarged prostate); the Propecia listing also has a 5mg option. At this dose finasteride slowly shrinks the prostate. It controls BPH rather than curing it, and it can take 6 months or more before passing urine gets easier. Every listing holds the same ingredient; what differs is the strength and the name on the box.
One tablet daily at a regular time, with or without food; a missed tablet is skipped. Reported effects include difficulty getting or keeping an erection, lower sexual desire, ejaculation problems with a smaller volume, and testicle pain. Changes in the breasts (lumps, soreness or discharge from the nipple), new or worsening depression and thoughts of self-harm, and allergic swelling of the lips or face are the serious ones. Finasteride may reduce fertility in men, can lower PSA readings, and has been linked with a higher risk of high-grade prostate cancer and of breast cancer.
Women are not treated with it: anyone who is or could be pregnant risks harming a male baby by touching damaged tablets. Tablets should be stored at room temperature, in a dry spot, and away from children.
