Hydrochlorothiazide is a thiazide diuretic — a water tablet that makes the kidneys pass more sodium into the urine. Water follows the sodium out, so there is less fluid in the circulation and less pressure pushing against the artery walls. That is the effect that brings blood pressure down, on its own or alongside another blood pressure medicine. The same fluid-clearing action helps with oedema: swelling from fluid held in the tissues, which can come with heart, kidney or liver problems, or from medicines such as corticosteroids and oestrogen. It also has a place in a rare thirst condition called diabetes insipidus, and in preventing kidney stones when blood calcium runs high.
Hydrochlorothiazide tablets come in 12.5mg and 25mg, both as tablets to swallow with water. The 25mg tablet holds twice as much of the medicine as the 12.5mg one, so the strength you choose follows the dose you take. It is one of the Heart & Blood Pressure medicines sold here. Once a day is the usual pattern, though some people split it into two doses, and for swelling it is sometimes used only on certain days of the week. Taking it in the morning keeps the extra toilet trips to daylight; with two doses a day, make the second one early afternoon rather than bedtime.
Two things are common at the start: more trips to the toilet, and a light-headed feeling when you stand up quickly. Rising in stages — sitting on the edge of the bed for a moment first — takes the edge off that. Alcohol makes the dizziness worse, so a heavy night shows more on this medicine. Both effects usually ease as the body adjusts. The effect on blood pressure builds gradually over the first few weeks rather than after a single tablet.
Headache, thirst, cramps, tiredness and dizziness come up most often. The tablet also changes the mix of salts in the blood, and a drop in potassium can bring weakness, cramp or a fluttering heartbeat. Confusion, a very dry mouth or far less urine than usual points to too little fluid. A blistering or peeling rash, or swelling of the face, lips or throat, needs urgent treatment.
Years of use bring two slower effects. The skin becomes more sensitive to sun, and some skin cancers become more likely, which makes sunscreen and a hat sensible outdoors. Blood sugar and uric acid may creep up too, a point for anyone with diabetes or gout.
Taking a daily painkiller such as ibuprofen or naproxen is the commonest clash, because it blunts the blood pressure drop. Lithium is the other to watch: hydrochlorothiazide pushes its level up. If you also take cholestyramine or colestipol for cholesterol, space the doses apart, since taken together they stop the tablet being absorbed. Alcohol and other blood pressure medicines pile onto the dizziness.

A drink is not off limits. The catch is that alcohol and hydrochlorothiazide both lower pressure, so standing up fast after a few drinks can make the room spin. Light drinking avoids most of that.
