Itraconazole is a triazole antifungal capsule in 100mg and 200mg, taken for fungal nail infections, ringworm, thrush and deeper fungal infections.
Fungi build their cell membranes from ergosterol, and itraconazole blocks an enzyme they need to make it. The membrane starts to leak and the fungus stops growing. The drug is fat-soluble, so it collects in skin, nails and mucous linings and stays there after the last capsule.
Toenail fungus is the usual reason to buy it. The same capsule is also used for ringworm and other tinea infections, for pityriasis versicolor (pale or brown scaly patches), and for thrush in the mouth, throat or vagina. At the far end sit deep infections such as histoplasmosis, blastomycosis and aspergillosis, usually breathed in from soil, dust or mould spores.
Itraconazole comes as capsules in 100mg and 200mg. Sporanox, the generic Sporanox listing, is the same ingredient in 100mg capsules. Choose by name and strength; the capsules in both listings are taken the same way. Itraconazole is also made as a liquid, which is absorbed differently and is taken on an empty stomach, whereas capsules go with a full meal.
Nails are where people go wrong. A treated nail grows out slowly and can look clean while fungus is still underneath; stop at that point and the infection is often back within months. Vaginal thrush, by contrast, may need only a short course, ringworm a little longer, and deep infections are treated for months.
Anyone on simvastatin for cholesterol cannot simply add itraconazole. The reason is CYP3A4, a liver enzyme that itraconazole strongly slows, so medicines that rely on it for breakdown build up. Lovastatin, oral midazolam and triazolam (sedatives), pimozide, quinidine, dofetilide (an antipsychotic and two heart-rhythm medicines) and methadone are on the same no-list. Blood thinners and some cancer medicines need adjusting, and acid reducers cut how much capsule gets absorbed.
The heart is the other limit: itraconazole can weaken its contractions, so heart failure rules it out. The liver can be hurt in some people. Day to day, expect nausea, stomach pain, diarrhoea, headache or a bad taste. It is avoided in pregnancy.

Sporanox is a brand name of itraconazole, which is also sold as Candiforce and Itrasys. Here, itraconazole can be bought online as generic Sporanox and itraconazole capsules. These are generics, not the original brand.
Both are triazole antifungals, but they do different jobs. Itraconazole also covers moulds such as Aspergillus and is the one usually used for nails, while fluconazole is more often used for yeast infections such as thrush. Itraconazole interferes with more medicines, because it blocks a liver enzyme more strongly.
Swallow them whole. Breaking the capsule open, chewing it or crushing it changes how the medicine is absorbed, so take it with a full meal and a glass of water instead.
Yes, for a while. After the last capsule the drug stays in skin and nails for weeks to months, so an infected nail can go on improving. Then the nail has to grow out, which is slow.
