Levothyroxine tablets replace the thyroid hormone missing in an underactive thyroid, in six strengths from 25mcg to 200mcg and packs of 100 to 400 tablets.
The thyroid, a small gland at the front of the neck, makes thyroxine (T4), the hormone that sets the pace of the body's metabolism. The body converts T4 into a more active form, T3, inside its tissues. When the gland slows down, most often because of the autoimmune condition Hashimoto's thyroiditis, or after thyroid surgery or radioactive iodine, T4 falls and everything runs slower.
Levothyroxine is synthetic T4, identical to the natural hormone. Taking it every day restores the missing supply, and the body makes its own T3 from it as usual. It is the first-line treatment for hypothyroidism worldwide and, once the dose is right, most people take the same strength for years with a blood test once or twice a year.

The active ingredient and strengths are the same. Levothyroxine has a narrow therapeutic range, meaning small changes in the amount absorbed can shift thyroid blood levels, so many people stay with one product and have their levels rechecked about six weeks after any change of brand or strength.
The gut absorbs levothyroxine best when it is empty, which is why it is taken in the morning, 30 to 60 minutes before breakfast, at the same time each day. A long list of things reduce absorption: calcium and iron supplements, antacids, acid-reducing medicines for heartburn, soya products and high-fibre meals. Calcium carbonate and ferrous sulfate in particular should be taken at least 4 hours apart from the tablet.
Too little hormone leaves the old symptoms in place: tiredness, cold intolerance, constipation and weight gain. Too much mimics an overactive thyroid, with a pounding or fast heartbeat, heat intolerance, tremor, sweating, anxiety and poor sleep. Older people and those with heart disease usually start on a low strength such as 25mcg and build up slowly. Chest pain or an irregular heartbeat while taking it calls for urgent assessment.

Synthroid and Levothroid are brand names of levothyroxine, which is also sold as Eltroxin, Euthyrox, Levoxyl, Thyronorm and Tirosint. Here, levothyroxine can be bought online as generic Synthroid and levothyroxine tablets. These are generics, not the original brand.
Symptoms usually start to ease within a few weeks, but the blood level of thyroid hormone takes about six weeks to settle on a new strength. That is why thyroid tests are usually repeated six to eight weeks after starting or changing the dose.
In an underactive thyroid, correcting the hormone level can reverse some of the weight gained from the condition, mostly retained fluid. In people with normal thyroid function it is not a weight-loss treatment, and taking extra can cause heart rhythm problems.
Some people take it at bedtime, at least three hours after the last meal, and absorption is similar. Choose one routine and keep to it, because switching back and forth changes how much is absorbed.
