Liothyronine is triiodothyronine, the thyroid hormone known as T3, made in a laboratory. The thyroid gland at the front of the neck normally produces it to control how quickly cells burn energy, and when the gland underperforms the whole body slows: tiredness, weight gain, feeling cold, dry skin, aching joints, low mood, heavy periods and a slow pulse. The tablets replace the hormone that is missing so the level returns to normal and those symptoms ease. It also has a place in some thyroid cancer treatment plans and where thyroid-stimulating hormone (TSH) needs to be pushed down.
Levothyroxine (T4) is the usual everyday choice for an underactive thyroid: the body keeps a store of it and converts it into T3 as required. Liothyronine skips that conversion because it already is T3. It therefore acts sooner and leaves the body sooner, and the level rises and falls more steeply across the day. That makes it the option when a quicker, more direct effect is wanted, and it is sometimes taken alongside levothyroxine.
Swallow the tablet with water, once a day, at the same time each morning. On an empty stomach, 30 to 60 minutes before breakfast, absorption is at its most reliable. Iron and calcium supplements, and antacids, can reduce how much gets through, so leave a gap of a few hours around them. Thyroid hormone also strengthens the effect of warfarin and other blood thinners, and it changes the way the body handles diabetes medicines and some heart medicines.
A well-matched dose usually goes unnoticed. When the dose runs too high the body behaves as if the thyroid were overactive: the heart pounds or races, hands tremble, sleep turns restless, and you feel hot, sweaty and on edge, often losing weight while eating normally, with looser stools. When it runs too low, the original underactive symptoms hang on. Long stretches on more hormone than the body needs can strain the heart and thin the bones, which is why blood test results, not how you feel that week, drive any change of dose. Chest pain or a fast, irregular heartbeat needs urgent treatment, and so does a swollen face or throat with difficulty breathing, which signals an allergy to the tablet.
