Nortriptyline sits in the tricyclic family of antidepressants, the older generation of medicines for low mood, and depression is still its main job. It is also used for nerve pain — the burning, shooting or aching discomfort that can outlast a bout of shingles by months or years (post-herpetic neuralgia). Panic disorder and help with giving up smoking are two further uses it has had. Nerve pain is usually treated with a smaller daily amount than depression.
Messenger chemicals called noradrenaline and serotonin carry signals from one nerve cell to the next; after release they are normally drawn back into the cell that sent them. Nortriptyline stops the cells pulling these messengers back in, leaving more of each in the gap between them, which strengthens the signalling from cell to cell. This is thought to be how the medicine lifts mood and alters the way pain is felt. Because the effect accumulates, weeks rather than days pass before mood improves, and nerve pain can take longer still. Amitriptyline, a close relative, is converted into nortriptyline inside the body.
Morning fog is the effect people mention first, and it is why many shift the whole daily amount to the evening; the day's amount can also be divided into smaller doses. Food does not change how the capsules work, so take them with a meal or in the gap between two — what matters is the same hour each day. The amount starts low and is raised gradually. A parched mouth and sluggish bowels are the other early complaints, often with blurred vision and a spell of light-headedness on getting up; for most people these settle within a few weeks. A few notice extra sweating, weight gain, or difficulty passing urine.
Some reactions need urgent treatment: a fast or uneven pulse, fainting, or swelling of the lips, face or throat after a dose. Mood needs watching when treatment starts and after any change in amount, and new thoughts of self-harm are a serious sign, not something to wait out.
Nortriptyline must never be taken with a MAO inhibitor. These are older antidepressants such as phenelzine and tranylcypromine, and some are also used in Parkinson's disease; together with nortriptyline they can send blood pressure dangerously high. Alcohol, strong painkillers and sleeping tablets all deepen the drowsiness. After a long course the amount is reduced step by step, because stopping suddenly can bring headache, nausea and disturbed sleep.
There are two packs: Nortriptyline, sold under the plain name, and Pamelor, the brand. Both hold 25mg capsules of the same medicine, so only the name on the box separates them. A daily amount above 25mg means taking more than one capsule. Capsules are the only form sold here, although nortriptyline also exists as an oral liquid; leave them in the strip until each dose is due, somewhere cool and dry.
