What pantoprazole treats
Pantoprazole belongs to the proton pump inhibitors, a group of medicines that turn down the amount of acid the stomach produces. It is used for gastro-oesophageal reflux disease (GORD), where stomach acid washes back up and causes heartburn and a sour taste, and for reflux oesophagitis, where that acid has left the lower oesophagus (the food pipe) inflamed and sore. It also protects the stomach in people taking non-steroidal anti-inflammatory painkillers (NSAIDs) long term who are at risk of ulcers, and forms part of combination treatment to clear Helicobacter pylori infection. This is a medicine for acid-related conditions, not a remedy for the occasional bout of indigestion after a large meal.
Taking pantoprazole tablets
A common routine is a single tablet with a glass of water before breakfast, then nothing else to remember until tomorrow.
Whole means whole. The tablet has a gastro-resistant (delayed-release) coating, and it is that coating which gets pantoprazole through the stomach and into the intestine intact, so a tablet that has been crushed, chewed or halved will not do its job.
Pantoprazole turns off many of the pumps that make acid, and the pumps run hardest at mealtimes, hence the dose beforehand. Acid output starts to drop with the first tablet. Heartburn and soreness take longer to ease, usually several days.
Side effects to expect
Headache, diarrhoea, stomach pain, wind, nausea and aching joints are the complaints that come up most, and they tend to ease as the body adjusts. Rarely, pantoprazole causes inflammation of the kidneys or a serious skin reaction; a spreading, blistering rash with fever needs urgent treatment. Long courses have been linked to low magnesium and low vitamin B12.
Medicines that don't mix well with it
Reducing stomach acid can affect how well some other medicines are absorbed. Itraconazole and ketoconazole, used for fungal infections, and atazanavir, used in HIV treatment, are the ones that matter most; high-dose methotrexate is affected as well. Those combinations are usually avoided or handled with a different dose.
