Two quite different conditions bring people to ropinirole. In Parkinson's disease the cells that make dopamine are lost, and limbs turn stiff, slow or shaky; ropinirole is a dopamine agonist, which means it stimulates the receptors that dopamine would normally fire. In restless legs syndrome an urge to move the legs creeps in while you rest, mostly late in the day or through the night. The tablets take the edge off both without curing either, so they tend to be a long-term arrangement.

Both Requip and Ropinirole tablets contain ropinirole, and both are ordinary tablets in four strengths: 1mg and 2mg for the higher steps, 0.25mg and 0.5mg for starting and fine-tuning. Requip carries the brand name; the other pack carries the ingredient name. Both sit in the neurology range.
How the day is arranged
Which dose pattern you follow depends on the condition. Parkinson's symptoms are usually treated with three doses spread through the waking day, so the level of medicine stays fairly steady. Restless legs syndrome is at its worst in the evening and overnight, so a single tablet is taken one to three hours before bed and works through the night. Swallow them with water; food makes little difference to how well they work, though taking a dose after a meal can settle nausea. Doses are raised in small steps, and several weeks can pass before the right amount is found.
Effects to expect
Headaches, dizziness and drowsiness come first for most people, and they usually fade once the dose has held steady for a while. Standing up quickly can bring a faint, woozy moment as blood pressure dips. In Parkinson's, especially at higher doses and in older people, seeing or hearing things that are not there is possible. Two effects catch people out. Sleep can arrive without warning in the middle of the day, at the wheel or halfway through a meal. And a compulsion to gamble, shop, overeat or pursue sex can build weeks or months after the start; it fades when the dose comes down or the medicine is stopped.
Coming off, and keeping the tablets
If ropinirole is to be stopped, the dose is lowered step by step. An abrupt stop can bring fever, sweating, stiff muscles and confusion, and that combination needs urgent treatment. The tablets stay in their original pack, in a dry place.
