Sertraline is an SSRI antidepressant sold here as 25mg, 50mg and 100mg tablets, taken once a day for depression, panic attacks and obsessive-compulsive disorder.
Depression is the best-known use, but sertraline is an SSRI (selective serotonin reuptake inhibitor) that is also used for obsessive-compulsive disorder (OCD), panic disorder, post-traumatic stress disorder, social anxiety disorder and premenstrual dysphoric disorder, the severe mood and physical symptoms that come before a period. Serotonin is a chemical messenger in the brain, and sertraline slows its reabsorption so more of it stays available between nerve cells; the effect builds over weeks rather than hours. Sertraline tablets come in 25mg, 50mg and 100mg and sit with the other antidepressants sold here.
Sertraline usually starts at 25mg or 50mg and goes up after a week or more if the response is only partial, which is where the 100mg tablet comes in. Morning or evening both work, with water and with or without a meal, so long as the time stays the same. A few weeks can pass before the full effect is clear. After weeks or months of use the dose is stepped down rather than dropped, because stopping outright can set off nausea, sweating, dizziness, headaches, poor sleep and a dip in mood.
Headaches, nausea, diarrhoea, dry mouth, dizziness and drowsiness top the list and usually ease within a couple of weeks. Sleep can go either way, sex drive often dips, and over a long course weight tends to creep up. Two rare reactions need urgent treatment: serotonin syndrome, where a fast heartbeat, sweating, shakiness, twitching muscles and confusion or agitation arrive together, and a severe allergic reaction with a swollen throat or tongue, an itchy raised rash and difficulty breathing. New or worsening thoughts of self-harm are a recognised risk, most often in people under 24 and in the first weeks of a course or after a dose change.
An MAO inhibitor — a different class of antidepressant, and some medicines for Parkinson's disease — is the combination to avoid completely, with at least 14 days between the two. Tramadol, migraine triptans and St John's wort also push serotonin up, and painkillers such as ibuprofen or aspirin, and warfarin, make bleeding in the stomach or gut more likely. Alcohol does not stop sertraline working, but it adds to the dizziness and drowsiness, especially in the first few weeks. Keep the tablets in their box, somewhere dry.

The first small changes often appear within a couple of weeks, but the full benefit takes a few weeks or longer, and sleep or appetite can shift before mood does. The dose is raised slowly, not more than once a week.
Alcohol is not off limits, but it tends to hit harder on sertraline: the drowsiness and dizziness can be stronger for a few hours afterwards, and a hangover can feel worse. The first few weeks tend to be the worst for this.
The number tells you how much sertraline one tablet holds. Courses usually begin at a low strength and step up only if the lower tablet is not doing enough, so a course can move from one tablet size to another over time.
Lustral is one of the brand names for sertraline, so the active ingredient is the same; only the brand on the box differs. The tablets sold here carry the ingredient name rather than a brand.
Sertraline is usually taken for months rather than weeks, and treatment often carries on for a while after you feel better instead of stopping as soon as mood lifts. How long depends on the condition and on how settled the symptoms have been.
