What topiramate is used for
Topiramate is an anticonvulsant — a medicine that calms the overactive nerve signalling behind seizures and migraines. Taken every day, it cuts the number of seizures in epilepsy and the number of attacks in people who get migraines often. It is a preventer, not a painkiller: it will not clear a migraine that is already under way.
How it works
The brain runs on a balance between signals that excite nerve cells and signals that quiet them. Topiramate pushes that balance towards the quieter side, which is why it is used where nerve cells fire too readily. Treatment starts at a low dose and is stepped up over a few weeks rather than starting high. Tablets go in once or twice a day, at around the same times, with or without food, and are swallowed whole — they taste bitter, and a broken tablet loses strength over time. Coming off topiramate suddenly can bring on seizures, even in someone who has never had one, so the dose is tailed off in steps instead.

Side effects worth knowing about

Pins and needles in the hands and feet, a flat or odd taste, shaky hands, low mood and weight loss are common too. Drowsiness matters if you drive or use machinery. Rarely, topiramate triggers a serious allergic reaction — a swollen throat or tongue, a raised itchy rash, trouble breathing — a blistering skin reaction, or a build-up of acid in the blood that shows as unusual sleepiness, no appetite and an irregular heartbeat. Sudden eye pain or blurred vision needs urgent treatment. Kidney stones are more likely on topiramate than off it.
Alcohol and hormonal contraception
Alcohol and other sedating medicines stack with the drowsiness, so a drink that would normally sit fine can hit harder. At higher doses topiramate can make the combined pill, the patch and the ring less reliable. Topiramate can also affect how a baby develops in pregnancy.
