What the capsules are used for
Cyclosporine settles an overactive immune system. It is a calcineurin inhibitor: it blocks a step inside T-cells — the white blood cells that coordinate immune attacks — so they stop driving inflammation against the body's own tissues or a donated organ.
The capsules are used in three situations: to stop the body rejecting a transplanted kidney, liver or heart; in rheumatoid arthritis; and in severe psoriasis. In the last two, cyclosporine is kept for people who have not done well on other treatments. Treatment is continuous rather than something taken only when symptoms flare.
How it is taken
Most people take cyclosporine twice a day, morning and evening. Doses are worked out from body weight and then fine-tuned with blood tests, because the level has to stay inside a narrow range: too low and the immune system starts up again, too high and the kidneys and blood pressure take the strain.
Swallow the capsules whole with water. Keep the timing steady — the same times each day and the same relation to meals, since food changes how much is absorbed. Store them in their blister pack, out of direct sun.
Side effects
Headache, nausea, stomach discomfort, shaky hands, swollen ankles, acne and extra hair growth are common and often ease as the body adjusts. Cyclosporine can also make the gums swell.
What matters more are the effects that build up quietly: reduced kidney function and blood pressure that stays high. Because immune defences are held down, infections take hold more easily, and years of treatment raise the risk of skin cancer and lymphoma. A rash with swelling of the face or throat and difficulty breathing needs urgent treatment.
Grapefruit and other medicines
Grapefruit and grapefruit juice push cyclosporine levels up, sometimes a long way, so leave them out while you are taking it. So do ketoconazole and itraconazole, clarithromycin and erythromycin, diltiazem and verapamil, and some HIV medicines. Rifampicin, carbamazepine, phenytoin and St John's wort push levels the other way. Ibuprofen and similar painkillers add to the load on the kidneys.
