Vsmall is vildagliptin in 50mg tablets, a medicine for type 2 diabetes in adults, used alongside diet and exercise. It belongs to the DPP-4 inhibitors, or gliptins. They block an enzyme that breaks down incretins, hormones the gut releases after eating, so the pancreas releases more insulin when blood sugar is high and the liver releases less sugar. Because the effect depends on raised blood sugar, vildagliptin on its own seldom causes low blood sugar. It is not for type 1 diabetes or for diabetic ketoacidosis.
How Vsmall is taken
Vildagliptin can be used alone or added to metformin, a glitazone, a sulfonylurea or insulin. The usual pattern is one 50mg tablet in the morning and one in the evening, up to two tablets a day; alongside a sulfonylurea it is one tablet in the morning. Vsmall has a single strength, 50mg, so it is the number of tablets, not the tablet size, that sets the daily amount. Reduced kidney function also brings the dose down to one tablet a day. Tablets are swallowed with water, with or without food. A missed tablet is taken when you remember it, but not doubled on the same day. Store at room temperature, dry.
Side effects
Dizziness, headache and constipation are the more usual ones, and ankle swelling can occur. Tremor, sweating and hunger are signs of low blood sugar, which is likelier when Vsmall is combined with a sulfonylurea or insulin. Persistent, severe stomach pain, sometimes with vomiting, can mean pancreatitis, and yellowing of the skin or eyes with dark urine points to the liver. Liver function is checked with blood tests during treatment. Blistering skin lesions and swelling of the face, lips or tongue are rare but need prompt attention.
