What orlistat does
Orlistat is the only medicine in this group that works entirely in the digestive tract. It sticks to the lipase enzymes that cut dietary fat into pieces small enough to absorb, and with those enzymes disabled, roughly 30 per cent of the fat you eat leaves the body unchanged. There is no effect on appetite and next to none in the bloodstream. The idea is simple: fewer calories get in from fat, and weight comes down gradually when the rest of the diet cooperates. It is used for weight management in adults with obesity.

Both products are capsules in the same two strengths, 60mg and 120mg.

Taking it with meals
A capsule goes with a main meal, or within an hour after it, up to three times in a day. No fat in the meal, or no meal at all, and the capsule stays in the box. Eating moderate, evenly spread fat helps, since a greasy dinner brings the worst of the side effects. Orlistat pushes out vitamins A, D, E and K along with the fat, so a multivitamin is usually taken at bedtime, well away from a dose. Store capsules at room temperature, dry and out of the heat.
Side effects and cautions
Expect digestive changes. Oily leakage, flatulence, soft or frequent stools and sudden urges are the typical ones, strongest in the first weeks and after high-fat food. Tummy pain is reported less often, and in rare cases the liver is affected: yellow skin, yellow eyes, dark urine or itching are the signs to look for. People with chronic malabsorption or cholestasis (when bile cannot flow properly) should not use it, and neither should anyone who is pregnant or breastfeeding.
Other tablets can be caught up in it. If you take ciclosporin, levothyroxine for your thyroid or amiodarone for your heart rhythm, orlistat changes how much of them you absorb. Warfarin users may find their clotting results move. And a bad bout of diarrhoea can stop the contraceptive pill being absorbed.

How long should I take orlistat for?
It is taken for as long as it helps and the diet stays lower in fat. The weight you lose is gradual and depends on your eating as well as the capsules, so there is no fixed length that suits everyone.
Can orlistat cause oily stools?
Yes, they are the best-known effect. Fat that is blocked from absorption leaves in the stool, which can look oily, leak, or arrive without much warning, especially after fatty meals. Lighter meals make it easier.
Is orlistat the same as Xenical?
Xenical is a brand name for orlistat, and it is the same ingredient. In the shop, the Xenical listing is generic orlistat, available in the same 60mg and 120mg capsules as the plain Orlistat listing.
Do I need vitamins while taking orlistat?
