Oseltamivir
Oseltamivir is the antiviral used against influenza A and B, to shorten the illness or prevent it after exposure; PockDose lists it as generic Tamiflu 75mg capsules.
Oseltamivir and influenza
Oseltamivir is an antiviral made for one job, influenza. Flu arrives suddenly, with fever, chills, aching muscles and joints, headache, a dry cough, a sore throat, a blocked or runny nose and a heavy tiredness that a cold rarely produces. Oseltamivir is aimed at the virus behind that picture, types A and B, and nothing else. Colds, COVID-19 and stomach bugs are different viruses and it does not touch them.
It is a prodrug. The liver converts the capsule's oseltamivir into its active form, which blocks neuraminidase, the enzyme that lets freshly made flu viruses leave the cell they grew in. Trapped viruses cannot go on to infect their neighbours, and the immune system gets time to catch up. It slows the virus; it does not kill it, and it does nothing against bacterial infections that sometimes follow flu.

Treatment, prevention and timing
Used for treatment, the benefit depends on speed. Started within two days of the first symptoms, it shortens the time that the fever, aches and congestion last. Left later, it has much less to offer. The treatment course is five days, with a capsule in the morning and another in the evening.
Used for prevention, it is a daily capsule for ten days at the least after contact with someone who has flu, or up to six weeks while an outbreak runs through the community. It protects only while it is being taken, and it does not replace the yearly flu vaccine.
Safety notes
Nausea is the effect most often noticed, followed by vomiting, stomach pain, diarrhoea and headache. Taking a capsule with food or milk usually settles the stomach. Rash, blistering, swelling of the face or tongue and trouble breathing are allergic signs. Confusion, odd behaviour and hallucinations turn up in people with flu, mostly children and teenagers, with or without the drug.
Kidneys remove oseltamivir, so reduced kidney function means a lower amount. Probenecid slows its removal. A dry shelf at ordinary room temperature is fine for the capsules.

Does oseltamivir cure the flu?
No. It shortens the illness and eases its length rather than ending it at once. The immune system still does the clearing, and treatment started later than two days after the first symptoms has far less effect.
Can oseltamivir stop me catching the flu?
While it is being taken, it can prevent flu in someone who has been in close contact with a case, which is the reason for the once-daily course. Once the capsules stop the protection stops too, and the vaccine remains the longer-term protection.
How many 75mg capsules do I need for a flu course?
For an adult, treatment is one capsule twice a day for five days, so ten capsules. Prevention is one a day, which makes ten capsules for ten days and about forty-two for the full six weeks, so the pack size follows the length of the course.
