Biltricide is praziquantel 600mg tablets for schistosomiasis (a blood fluke infection) and liver fluke infections, supplied in packs from 4 to 80 tablets.
Praziquantel is an anthelmintic, a medicine that kills parasitic worms. It is aimed at flatworms: the blood flukes that cause schistosomiasis and the liver flukes that live in or near the liver, bile ducts included. Schistosomiasis starts when larvae released by freshwater snails burrow through skin during contact with infested water. The adult worms settle in blood vessels, and the eggs they shed lodge in tissue, where the inflammation does the lasting harm. Killing the worms ends that cycle. A stool, urine or blood test usually confirms the infection first. The disease is widespread in tropical and subtropical regions, and praziquantel also treats other worm infestations, tapeworm among them.
A typical day starts with breakfast: the first dose goes down with water alongside that meal, the second follows four to six hours later with lunch or an early dinner, and the third four to six hours after that. Eating matters here, since the tablets absorb better with food in the stomach. The bitter taste is the main complaint, so each tablet is swallowed quickly instead of being held in the mouth, where it can make you gag or bring on vomiting. Infections that need a longer or repeated course follow the schedule printed with the pack.
Headache, dizziness, stomach pain, nausea, fever, itching and a general washed-out feeling are the most common effects, and they pass. A raised, itchy rash that spreads (hives) is the exception and needs urgent treatment.
Rifampicin, an antibiotic used for tuberculosis, lowers praziquantel levels so much that the infection will not clear, so rifampicin is stopped four weeks before the course and restarted the day after it ends. Cimetidine, a heartburn medicine, does the opposite and raises praziquantel levels. Until the course begins, the tablets stay somewhere cool, away from heat.
