Depression and generalised anxiety — worry that runs on most days and is hard to switch off — are its best-known uses. It also has a place in pain that outlasts an injury: the tingling, burning or numb hands and feet of diabetes-related nerve damage, fibromyalgia, and long-standing back or knee osteoarthritis pain. Capsules are made in four strengths: 20mg, 30mg, 40mg and 60mg.
How duloxetine works
Messages cross from one nerve to the next through a tiny gap. Serotonin and noradrenaline are released into that gap, then drawn back into the nerve that sent them. Duloxetine blocks that draw-back, so both messengers linger longer. Over weeks this is thought to steady the signalling behind mood and worry and to blunt pain signals on their way up to the brain. Expect the shift to arrive over weeks, not overnight.
Taking the capsules
A Cymbalta capsule is a shell packed with tiny coated beads, and the coating is the reason it must be swallowed whole with water. Chewed, split or emptied out, the beads release duloxetine in the stomach instead of further down the gut. Meals are irrelevant. The usual start is a low strength, raised after a week or two, taken as one capsule a day or divided across two.
Side effects to expect
Dry mouth, nausea, headache, sleepiness or dizziness, loose stools or constipation, sweating, broken sleep and a dip in sex drive are the usual early complaints; most ease off over the first fortnight. Hold off on driving or machinery until you know how the capsules affect you.
