What Cytoxan treats
Cytoxan is a brand name for cyclophosphamide, one of the oldest chemotherapy medicines and still a widely used one. It belongs to the alkylating group, sometimes called nitrogen mustards: once inside the body it is converted into active forms that attach to DNA in dividing cells, so those cells can no longer copy themselves. That double action explains its two jobs. It is used for cancers that grow from white blood cells, such as lymphoma and leukaemia, and for some solid tumours. It is also used as immunosuppression — damping down an overactive immune system — in conditions such as lupus nephritis (kidney inflammation caused by lupus) and vasculitis (inflamed blood vessels).
The daily tablet, and why fluids matter
The tablets are 50mg and are usually taken as a set number of tablets once a day. Food makes no difference, so the dose can go with breakfast or on its own. What does matter is water. Cyclophosphamide is broken down into a substance that can irritate the lining of the bladder, and drinking steadily through the day, then emptying your bladder when you need to, keeps that irritation down. A dose taken earlier in the day suits this better than one taken at bedtime. Blood counts are checked regularly during a course, because the medicine lowers the number of white cells that fight infection and of platelets that help blood to clot.
Side effects: common ones, and signs that need attention
Nausea, vomiting and going off food are the usual complaints, often a few hours after a dose. Hair thins or falls out on many courses and grows back once treatment stops. Tiredness, mouth ulcers, diarrhoea and darkened skin or nails also turn up. With fewer white cells around, a fever, chills or a sore throat is a sign of infection and needs urgent treatment. Blood in the urine means the bladder is inflamed. Unusual bruising or bleeding, or breathlessness, are further signs that something is wrong.
Other medicines, and where to keep the tablets
Store them in their original carton in a cupboard that stays cool and dark. Because the liver switches cyclophosphamide on, anything that changes liver enzymes — St John's wort is the common example — can change how much active medicine the body produces. Cyclophosphamide also adds to the immune-damping effect of other treatments of the same kind, and it can strengthen the effect of warfarin, so bleeding becomes more likely.
