Nexium 20mg and 40mg tablets
Nexium here is generic esomeprazole, a proton pump inhibitor, in 20mg and 40mg tablets. Your stomach lining has small pumps that push acid into the stomach; esomeprazole shuts many of them down, so less acid is made and an irritated food pipe or ulcer gets time to heal. Esomeprazole is the S-isomer of omeprazole, meaning one of the two mirror-image halves of that molecule.
It is used for gastro-oesophageal reflux disease (acid washing back up into the food pipe), for healing erosive oesophagitis, for heartburn that keeps coming back, and for stomach and duodenal ulcers. It is also given with antibiotics to clear Helicobacter pylori, a bacterium behind many ulcers, to prevent ulcers in people on long-term anti-inflammatory painkillers, and for Zollinger-Ellison syndrome, a rare condition that makes the stomach produce too much acid.
20mg or 40mg
The 20mg tablet is the usual strength for reflux symptoms and heartburn. The 40mg tablet is used for healing erosive oesophagitis and for some ulcer courses. Oesophagitis is typically treated for four to eight weeks.
How to take it
One tablet once a day, swallowed whole with water and not crushed or chewed, ideally at least an hour before a meal. Relief builds up over a day or two instead of arriving on the first dose, so it is not a fast antacid. Store the tablets at room temperature, away from heat and moisture.
Side effects and interactions
Headache, diarrhoea, nausea, wind, stomach pain, constipation and a dry mouth are the usual ones. Taken for a year or more, esomeprazole can lower magnesium (muscle spasms, tiredness, an uneven heartbeat) and vitamin B12. Watery diarrhoea that will not stop could be a gut infection, Clostridioides difficile, which these medicines make more likely. Clopidogrel, the HIV medicines atazanavir and rilpivirine, methotrexate and the antifungals ketoconazole and itraconazole are all affected.
