Quitting with varenicline
Varenicline is a stop-smoking tablet with no nicotine in it. It attaches to the same receptors in the brain that nicotine uses, but switches them on only partly. That is enough to ease cravings and withdrawal, and because the receptors are already occupied, a cigarette gives much less of a reward if you slip. It is sold here as varenicline tartrate in 0.5mg and 1mg tablets, the same ingredient as Champix and Chantix.
How a course goes
You pick a quit date and start the tablets about a week before it, so you can still smoke at first. The usual build-up is:
- days 1 to 3: 0.5mg once a day
- days 4 to 7: 0.5mg twice a day
- from day 8: 1mg twice a day
That is why there are two strengths. Take each tablet after eating, with a full glass of water. A course is 12 weeks, and people who have stopped by then can go on for another 12 to hold on to the gain. In severe kidney disease the top dose is lowered to 0.5mg twice a day. Store the pack at room temperature, away from damp.
What does it feel like?
Nausea is the one almost everyone mentions, mostly in the first weeks, and it is milder if the tablet goes down after food with a full glass of water. Then there are the dreams: vivid, sometimes strange. Some people sleep badly, get headaches or constipation, or notice food tastes different. If it makes you dizzy or drowsy, do not drive. And be careful with alcohol: a few people find one drink hits much harder than it used to, or they behave out of character.
Giving up nicotine can itself leave you low and irritable, but agitation, hostility, a depressed mood or thoughts of harming yourself while on varenicline mean stopping the tablets and getting help at once. Seizures, swelling of the face or tongue and blistering skin reactions are rare and need urgent treatment.
