What Clomid tablets are for
Clomid is clomiphene citrate in tablet form, taken to bring on ovulation — the release of an egg from an ovary — in women who want to become pregnant but are not ovulating regularly, or at all. Infertility has many causes; clomiphene is used when the problem lies with ovulation itself, and it is a common first choice in polycystic ovary syndrome (PCOS), where cycles are long and unpredictable.
The tablets act on the brain, not the ovary. Clomiphene blocks oestrogen receptors in the hypothalamus (the brain's hormone control centre), so the brain reads oestrogen levels as low and signals the pituitary gland to release more follicle-stimulating hormone and luteinising hormone. Those two hormones push a follicle in the ovary to ripen and release its egg.
How a course is taken
The first course is usually one 50mg tablet a day for five days. If a period has started, or bleeding has been brought on, those five days begin on or about day five of the cycle; when there has been no recent bleeding at all, the course can start straight away. Take the tablet with water and keep to roughly the same time each day.
If a course does not produce ovulation, a later one can run at a higher daily dose — 100mg for five days. Courses are spaced a cycle apart rather than repeated back to back.
Side effects to expect
Hot flushes, pelvic heaviness or bloating, tender breasts, headache, nausea and low mood are the ones women notice most, and they usually settle after the five days are over. Blurred vision or spots before the eyes are uncommon; they are a reason to stop the tablets.
A few women over-respond to the medicine, and their ovaries swell as fluid builds up. Severe pelvic pain, an abdomen that swells quickly and shortness of breath need urgent treatment. The condition is called ovarian hyperstimulation syndrome.
