Revatio is the name sildenafil goes under when it is used for pulmonary arterial hypertension (PAH), the condition where pressure stays too high in the arteries running from the heart to the lungs. Narrowed vessels make the right side of the heart push harder to move blood through, which shows up as breathlessness and tiredness on ordinary effort. Sildenafil widens those vessels and eases the strain. The tablets are 20mg, the strength used for PAH.
How sildenafil works and how it is taken
It belongs to the PDE5 inhibitor group: it blocks an enzyme, phosphodiesterase type 5, that would otherwise clear away a chemical the body uses to relax the muscular walls of blood vessels. Leave more of that chemical in place and the lung arteries stay wider, so blood meets less resistance.
For PAH the dose is 20mg three times a day, spaced roughly four to six hours apart, with or without food. This is a fixed routine rather than a tablet taken when you feel breathless. A fatty meal slows absorption, and grapefruit juice pushes the level in the blood up, so it is simplest to skip the juice while taking it. Keep the strips in their box somewhere cool and dry.
Side effects worth knowing about
Headache and flushing are the usual ones, along with indigestion, a blocked or runny nose and dizziness. Some people get diarrhoea or nosebleeds. Most of these settle as the body gets used to the medicine.
A sudden loss of vision or hearing, ringing in the ears, or an erection lasting more than four hours all need urgent treatment. So does swelling of the lips, tongue or throat, or a rash spreading across the body.
Nitrates and other medicines to keep apart
Nitrates for chest pain (GTN spray or tablets, isosorbide) and amyl nitrite cause a dangerous drop in blood pressure when combined with sildenafil. Riociguat, another medicine used for the same lung condition, should not be combined with it either. Alpha-blockers used for blood pressure or an enlarged prostate add to the fall in pressure, and ketoconazole, ritonavir and similar drugs slow sildenafil's breakdown, so more of it stays in the body.
Revatio, Assurans and the higher-strength tablets
Revatio is one of two sildenafil 20mg tablets sold here for PAH; Assurans is the other, at the same strength. Everything else with sildenafil in it, including Cenforce, Kamagra, Viagra and their soft, chewable, capsule and jelly versions, comes at 50mg and above and is sold for erectile dysfunction, taken before sex rather than three times a day.
