What sildenafil does
Sildenafil, approved in 1998, was the first tablet for erectile dysfunction. It belongs to the PDE5 inhibitors. PDE5 is an enzyme that switches off the chemical signal telling blood vessel walls to relax; by blocking it, sildenafil keeps that signal going during arousal, so blood fills the penis more easily and the erection holds. Desire still has to come from you, and outside sexual activity the medicine does nothing noticeable.
A lower 20mg strength has a second use: pulmonary arterial hypertension, which is high blood pressure in the arteries of the lungs. Here the same widening effect lowers the pressure in those arteries. Revatio and Assurans are the 20mg listings for that condition, taken on a regular schedule rather than before sex.

Taking it and what to expect
For erections, a dose goes in about an hour before sex, at most once a day, and works for roughly the next 4 hours. Food matters: after a large, fatty meal the effect comes later. Headache, flushing, an upset stomach, a stuffy nose, dizziness and a short-lived blue tinge to vision are the usual complaints, and they are more frequent at higher strengths.
Never take sildenafil if you use a nitrate for angina, such as a GTN spray or isosorbide tablets, or if you use poppers or riociguat: blood pressure can drop far enough to make you faint, or worse. Doubling up with tadalafil or vardenafil carries the same danger. Rarely, an erection refuses to subside; after 4 hours it is called priapism and needs urgent treatment before the tissue is damaged.
