What tadalafil does
Tadalafil is used in two situations: erectile dysfunction (ED, trouble with erections) and the urinary trouble that comes with an enlarged prostate, known as benign prostatic hyperplasia (BPH). For ED it works as a PDE5 inhibitor. PDE5 normally clears away the signal that keeps penile blood vessels relaxed, and tadalafil holds it back, so stimulation produces a firmer erection. What sets it apart is how slowly the body gets rid of it. The half-life is about 17.5 hours, so it stays active far longer than sildenafil or vardenafil.
As needed or every day
Taken as needed, tadalafil is usually used at least 30 minutes before sex and not more than once in 24 hours, and one tablet's effect can last as long as 36 hours. Taken daily, a low-strength tablet goes in at about the same time each day, and sex can happen at any point between doses. Food makes no difference.
Safety and interactions
Reported effects are headache, indigestion, flushing, nasal congestion, back pain and muscle aches. Priapism (an erection beyond four hours) and sudden loss of sight or hearing are rare but need urgent treatment. Nitrates, "poppers" and riociguat must be kept away from tadalafil, as blood pressure can crash. Alpha-blockers and blood pressure tablets can lower it further. Ketoconazole, itraconazole, clarithromycin and ritonavir raise tadalafil levels, rifampicin lowers them, and heavy alcohol or large amounts of grapefruit juice add to the risk of dizziness and side effects.
